How Telehealth-Enabled Care Works
Both the CMS GUIDE Dementia Care Model and the upcoming CMS ACCESS Model were explicitly designed by Medicare to use telehealth, remote monitoring, and connected devices. This means seniors anywhere in Florida can access the same board-certified geriatrician-led team care as patients who live near our Loxahatchee office. We coordinate with your local primary care physician, specialists, and hospital systems — adding geriatric specialty expertise as an additional layer, not replacing your existing providers.
