Florida Elder Care is part of the first ACCESS cohort nationwide, and enrollment is open now for Medicare beneficiaries in Fort Lauderdale, Hollywood, Pembroke Pines, Coral Springs, Weston, and throughout Broward County. The program pairs remote monitoring and telehealth visits with outcome-aligned team care across all four tracks: early cardio-kidney-metabolic conditions (high blood pressure, cholesterol, prediabetes, weight), diabetes, kidney disease & heart disease, chronic knee, hip, and back pain, and depression or anxiety.
Services come at low or no cost, you keep your own Broward doctors, and Medicare pays us in full only when your health measurably improves.
