WHAT IS THE GUIDE MODEL?
The GUIDE (Guiding an Improved Dementia Experience) Model is a CMS Innovation Center initiative that provides comprehensive, coordinated dementia care. Florida Elder Care is one of the select practices in Florida participating in this groundbreaking program, which launched July 1, 2024.
The model is designed to improve quality of life for people living with dementia while reducing strain on caregivers — all at no additional cost to the Medicare beneficiary.
PROGRAM BENEFITS

Board-certified geriatrician oversight with ARNP, Care Navigator, and interdisciplinary team members coordinating all aspects of your care.

Your personal point of contact who coordinates services, connects you with community resources, and ensures continuity of care.

Round-the-clock access to clinical support for urgent questions, behavioral concerns, and caregiver crisis situations.

Up to $2,500 per year in respite services to give caregivers essential breaks, including in-home care, adult day services, and short-term facility stays.
🛡️
Up to $1,000 for home safety assessments and modifications including grab bars, door alarms, and fall prevention measures.
📋
Personalized care plans developed with the patient and caregiver, addressing medical needs, behavioral symptoms, safety, and future planning.
ELIGIBILITY
Medicare beneficiaries with a confirmed dementia diagnosis (Alzheimer’s disease or related dementias) who have traditional Medicare (Part A and Part B) and live in our service area. Patients cannot be enrolled in Medicare Advantage, PACE, or hospice, and cannot be in long-term nursing home placement. The program is available at no additional cost.
