The CMS ACCESS Model Is Coming to Central Florida

In July 2026, Florida Elder Care launches the CMS ACCESS Model across all four tracks — bringing technology-enabled chronic care to Medicare beneficiaries in Orlando, Winter Park, Kissimmee, Sanford, Clermont, and throughout Orange, Seminole, Osceola, and Lake counties. The program pairs remote monitoring and telehealth visits with outcome-focused team care for seniors managing hypertension, diabetes, chronic kidney disease, chronic knee, hip, and back pain, and depression or anxiety.
