What is a geriatrician?

A geriatrician is a physician who specializes in the healthcare of older adults. Beyond standard internal medicine training, geriatricians complete additional fellowship training focused on managing multiple chronic conditions, cognitive decline, medication interactions, fall prevention, and the complex health needs that come with aging. Dr. Manoj Bhattarai is triple board-certified in Geriatrics, Internal Medicine, and Nephrology.

What areas do you serve?

Our office is located at 13005 Southern Boulevard, Unit 225, Loxahatchee, FL 33470. We serve patients across Palm Beach County including Loxahatchee, Wellington, Royal Palm Beach, West Palm Beach, Palm Beach Gardens, Boynton Beach, and surrounding communities. We also provide geriatric house calls for patients who have difficulty traveling to our office.

Do you accept Medicare?

Yes. We accept traditional Medicare (Part A and Part B). Our CMS GUIDE Model services for dementia care are provided at no additional cost to Medicare beneficiaries. Please call our office at (561) 685-1522 to verify your specific coverage.

Do you make house calls?

Yes. We provide geriatric house calls for patients who have difficulty traveling to our office, including those in private homes, assisted living facilities, and memory care communities across Palm Beach County. House calls include the same comprehensive assessments and ongoing care management available at our office location.

CMS GUIDE MODEL

What is the CMS GUIDE Model?

The GUIDE (Guiding an Improved Dementia Experience) Model is a CMS Innovation Center program that provides comprehensive, team-based dementia care for Medicare beneficiaries at no additional cost. It includes a dedicated Care Navigator, 24/7 support line, up to $2,500 per year in respite care for caregivers, caregiver education, and coordinated medical management. Florida Elder Care is one of only 34 GUIDE providers in the state of Florida.

Who is eligible for the GUIDE Model?

To qualify, you must have a confirmed dementia diagnosis (Alzheimer’s or related dementias), have traditional Medicare (Part A and Part B), and live in the community (not in a long-term nursing facility). You cannot be enrolled in Medicare Advantage, PACE, or hospice. There is no age requirement beyond having Medicare eligibility.

Does the GUIDE Model cost anything?

No. All GUIDE Model services are provided at no cost to the patient or their family. There are no copays, deductibles, or out-of-pocket expenses for GUIDE services. CMS pays the practice directly to provide these comprehensive care services.

What is respite care and how does it work under GUIDE?

Respite care provides temporary relief for unpaid caregivers (typically family members) by covering the cost of substitute care. Under the GUIDE Model, caregivers receive up to $2,500 per year that can be used for in-home aides, adult day center services, or short-term facility stays. Your Care Navigator helps arrange respite services and manage the benefit.

How do I enroll in the GUIDE Model?

Call our office at (561) 685-1522 or fill out the contact form on our website. Your primary care physician, neurologist, or any specialist can also refer you directly. We handle all enrollment paperwork and can typically begin services within one to two weeks of initial contact.

Can I keep my current primary care doctor if I enroll in GUIDE?

Yes. The GUIDE Model is a supplemental care program — it adds a geriatrician-led dementia care team on top of your existing medical providers. Your primary care doctor, specialists, and other healthcare providers remain part of your care. Our Care Navigator coordinates with all your providers to ensure seamless, integrated care.

CMS ACCESS MODEL

What is the CMS ACCESS Model?

The ACCESS (Advancing All-Payer Health Equity Approaches to Chronic Care through Enabling Systems and Services) Model is a 10-year CMS innovation initiative launching July 2026. It focuses on technology-enabled chronic disease management for conditions including hypertension, diabetes, chronic kidney disease, chronic musculoskeletal pain, and behavioral health (depression and anxiety). Florida Elder Care is a participating practice.

When does the ACCESS Model launch?

The ACCESS Model launches in July 2026. We are currently preparing our systems, technology infrastructure, and care pathways. If you are interested in participating when the program begins, contact our office to be placed on our notification list.
