The CMS ACCESS Model Is Live in Miami-Dade
Florida Elder Care is part of the first ACCESS cohort nationwide, and enrollment is open now for Medicare beneficiaries in Miami, Coral Gables, Kendall, Aventura, Hialeah, and throughout Miami-Dade County. The program combines remote monitoring, telehealth visits, and outcome-aligned team care across all four tracks: early cardio-kidney-metabolic conditions (high blood pressure, cholesterol, prediabetes, weight), diabetes, kidney disease & heart disease, chronic knee, hip, and back pain, and depression or anxiety.
Services come at low or no cost, you keep your own Miami-Dade doctors, and Medicare pays us in full only when your health measurably improves.
