ABOUT THE MODEL
The ACCESS Model is CMS’s newest innovation initiative, designed to improve outcomes for patients with chronic conditions through technology-enabled care delivery and outcome-aligned payments (OAPs). Unlike traditional fee-for-service, ACCESS rewards providers for achieving measurable health improvements.
Florida Elder Care has been selected to participate in this 10-year program starting July 2026, bringing cutting-edge chronic disease management to seniors across Florida — from our headquarters in Loxahatchee to Miami-Dade, Broward, Orlando, Tampa Bay, Jacksonville, and every major Florida market.

Blood pressure management with remote monitoring, medication optimization, and lifestyle coaching to reduce cardiovascular risk.

Comprehensive diabetes care including glucose monitoring, A1C management, nutrition guidance, and complication prevention.

Non-opioid pain management strategies, physical therapy coordination, and functional improvement programs for chronic pain conditions.

Integrated treatment for depression and anxiety, including screening, therapy coordination, medication management, and wellness support.
