When to Refer to a Geriatrician
Consider a geriatric referral when your patient presents with multiple comorbidities requiring coordinated management across specialties, polypharmacy with five or more medications and potential drug interactions, new or worsening cognitive decline requiring comprehensive evaluation, recurrent falls or functional decline affecting independence, frequent hospitalizations or ER visits suggesting care fragmentation, caregiver burnout or family members struggling to manage care at home, need for comprehensive geriatric assessment before major surgical decisions, or complex goals-of-care and advance care planning discussions.

CMS GUIDE Model
For patients with confirmed dementia on traditional Medicare. Provides care navigator, 24/7 support, $2,500/yr caregiver respite, and comprehensive dementia care management at no cost to the patient. Florida Elder Care is 1 of 34 GUIDE providers in Florida.
Eligibility: Dementia diagnosis + traditional Medicare (no MA) + community-dwelling (no LTC nursing home) + not enrolled in PACE or hospice
CMS ACCESS Model
Launching July 2026. Technology-enabled chronic disease management for hypertension, diabetes, CKD, chronic MSK pain, and behavioral health. Outcome-aligned payments with remote monitoring and telehealth capabilities.

Geriatric House Calls
Comprehensive in-home geriatric assessments and ongoing care for homebound patients, those in ALFs and memory care, or patients who have difficulty traveling. Covered by Medicare. Serving all of Palm Beach County.
Concierge Medicine
Premium geriatric concierge services for assisted living and memory care facilities. 24/7 physician access, extended on-site visits, proactive wellness management, and seamless care coordination at no additional cost.

About Our Practice
Florida Elder Care is led by Dr. Manoj Bhattarai, a triple board-certified physician in Geriatrics, Internal Medicine, and Nephrology with fellowship training at Brown University and the University of Pittsburgh. Dr. Bhattarai has been serving Palm Beach County since 2016 and holds medical staff privileges at HCA Florida Palms West Hospital, Wellington Regional Medical Center, HCA Florida JFK Hospital, Bethesda Hospital, and Lakeside Medical Center.
Our practice is one of the select healthcare organizations in Florida participating in both the CMS GUIDE Dementia Care Model and the upcoming CMS ACCESS Chronic Care Model. We also serve as an Assistant Professor of Geriatrics at Lakeside Medical Center, mentoring the next generation of physicians in geriatric medicine.
