Depression and anxiety are common in later life — and highly treatable. Yet most older adults never receive consistent mental health care, whether because of stigma, transportation, or simply not knowing where to start. Medicare’s ACCESS Model Behavioral Health track changes that: it funds ongoing, technology-supported care for depression and anxiety that comes to you.

What the Program Includes

  • Regular check-ins by phone or video with a clinical team that knows you
  • Measurement-based care — we track your symptoms over time against your own baseline, so treatment adjusts when it isn’t working
  • Medication management by a geriatric team that understands how antidepressants interact with the other medications seniors take
  • Digital coaching and self-management tools designed for older adults
  • Coordination with your own doctor through structured Care Updates

Built for Older Adults

Late-life depression often looks different — fatigue, poor sleep, memory complaints, loss of interest — and frequently travels with chronic illness, grief, or caregiving stress. Our geriatrician-led team treats the whole picture, and under Medicare’s outcome-aligned payments we earn full payment only when your symptoms measurably improve. Participation is voluntary, low or no cost, and you may unalign at any time.
