The CMS ACCESS Model is Medicare’s new 10-year program for ongoing, technology-supported chronic care — remote monitoring, telehealth visits, coaching, and medication support — delivered at low or no cost to you. Florida Elder Care is among the first cohort of participants nationwide, serving patients across Florida since July 2026.
Who Is Eligible?
You may qualify if you are enrolled in Original Medicare and have at least one qualifying condition:
- High blood pressure, high cholesterol, prediabetes, or overweight/obesity — eCKM track
- Diabetes, chronic kidney disease (stage 3a/3b), or heart disease — CKM track
- Chronic musculoskeletal pain (back, joint, neck) — MSK track
- Depression or anxiety — BH track

How to Enroll — Three Ways

- Online: submit the form on our contact page and our team will call you back within one business day.
- Ask your doctor: any physician can refer you — and Medicare pays your doctor to stay involved in your care through the model’s co-management updates.
What Does It Cost?
ACCESS services come at low or no cost. As a participating organization, we can waive co-payments and deductibles for outcome-aligned payment services as a standard beneficiary engagement incentive. You keep every standard Medicare right and benefit you have today.
Your Rights in ACCESS
- Participation is 100% voluntary
- You keep your own primary care doctor and specialists — ACCESS adds support alongside them
- You may unalign from the program at any time, for any reason
- Our payment is tied to your results — we earn full payment only when your health measurably improves
