Medicare’s ACCESS Model now pays for continuous, technology-supported care to intervene before those diseases take hold.

  • Dyslipidemia (high or abnormal cholesterol/lipids)
  • Overweight or obesity with central adiposity
  • Prediabetes

How the Program Works

You receive connected, FDA-authorized monitoring devices (such as a blood pressure cuff and scale), regular telehealth check-ins with our clinical team, lifestyle and nutrition coaching, and medication management — all coordinated with your own primary care doctor, who receives structured Care Updates on your progress. Our targets track control or improvement in blood pressure, lipids, weight, and HbA1c against your own baseline — and Medicare pays us in full only when you improve. That is the meaning of outcome-aligned payment: our incentives are your outcomes.
