Chronic Musculoskeletal Pain Care for Seniors — ACCESS MSK Track

Chronic back, neck, and joint pain is one of the most common — and most under-treated — problems for adults 65+. Too often the choice seems to be living with it or relying on pills. Medicare’s ACCESS Model MSK track funds a third path: structured, technology-supported musculoskeletal care delivered where you live.

What the MSK Program Includes

  • Guided therapeutic exercise programs designed for seniors, delivered through easy-to-use digital tools
  • Regular telehealth check-ins to track pain and function against your own baseline
  • Medication review by a geriatrician — minimizing risky pain medications where safer options work
  • Coaching on pacing, sleep, and activity so improvement holds over time
  • Structured Care Updates to your own primary care doctor or orthopedist

Paid on Your Progress

Under outcome-aligned payments, Medicare pays us in full only when your pain and function measurably improve. Falls risk, frailty, kidney function, and drug interactions all change how senior pain should be managed — our geriatrician-led team builds the plan around those realities, not around a one-size-fits-all app.

Get started: enroll in ACCESS or call (561) 685-1522. Physicians: refer a patient. Program overview: CMS ACCESS Model.
