For PCPs, cardiologists, nephrologists, orthopedists, and behavioral health clinicians across Florida. The CMS ACCESS Model is a new 10-year Innovation Center model that pays for technology-enabled chronic care — and it was designed to work with your practice, not around it. Florida Elder Care is a first-cohort ACCESS participant, live since July 2026.
What Referring Clinicians Gain
- A new billable service: CMS created a co-management payment (CMP) that referring clinicians can bill for documented review of ACCESS Care Updates plus a related care coordination activity — such as a medication adjustment or problem list update. No beneficiary cost-sharing applies to co-management payments.
- Enhanced payment for enrollment assistance: clinicians who help a patient get set up may add a modifier to CMP billing for an enhanced payment.
- Structured, FHIR-based Care Updates: you receive concise electronic updates at care initiation and at meaningful clinical milestones — not another portal to log into.
- Your patient stays yours: ACCESS complements traditional care. Patients keep their PCP and specialists, and retain all standard Medicare rights.
Which Patients Qualify
Medicare beneficiaries with a qualifying condition in one of four clinical tracks:
- Early Cardio-Kidney-Metabolic (eCKM) — hypertension, dyslipidemia, overweight/obesity with central adiposity, prediabetes
- Cardio-Kidney-Metabolic (CKM) — diabetes, chronic kidney disease (stage 3a/3b), atherosclerotic cardiovascular disease
- Musculoskeletal (MSK) — chronic musculoskeletal pain
- Behavioral Health (BH) — depression and anxiety
Why Refer to Florida Elder Care
Our practice is led by a physician triple-boarded in Geriatrics, Internal Medicine, and Nephrology — direct specialty depth for the CKM and eCKM tracks that most technology-only ACCESS entrants cannot match. We already operate under CMS Innovation Center accountability in the GUIDE Model, where our patients have 10% fewer ER visits and 8% fewer hospitalizations than the national average (see the published CMS Q4 2025 data). Outcome-aligned payment means we only earn full payment when your patient measurably improves.
