Every year, Medicare offers seniors a benefit that costs nothing, takes about an hour, and can catch serious problems — including early memory loss — before they become crises. Here’s what an Annual Wellness Visit actually includes, how it differs from a physical exam, and why it matters so much for adults 65 and older.
What Is a Medicare Annual Wellness Visit?
The AWV is a yearly appointment covered by Medicare Part B, focused on prevention and planning rather than treating a specific illness. If you’ve had Part B for longer than 12 months, you’re eligible for one AWV every 12 months. (In your first year on Part B, you get a one-time “Welcome to Medicare” visit instead.)
When your provider accepts Medicare assignment, the AWV comes at no cost to you — no deductible and no coinsurance for the visit itself. If your provider addresses additional medical issues during the same appointment, those services may be billed separately, but the wellness visit components are free.
An AWV Is Not a Physical Exam — It’s Something Different
A common misconception is that the AWV is a head-to-toe physical. It isn’t. Think of a physical exam as checking the engine, while the AWV is more like sitting down with a mechanic to review the full maintenance history and plan the year ahead. The AWV is a structured review of your health risks, your medications, your function, and your future — designed to create a personalized prevention roadmap.
What’s Included in an Annual Wellness Visit
- Health risk assessment — a structured questionnaire covering your health status, behaviors, and risk factors
- Review of medical and family history — updated every year as things change
- Routine measurements — height, weight, blood pressure, and body mass index
- Cognitive assessment — a required screening to detect early signs of memory loss or dementia
- Depression and mood screening — because mood changes in older adults are common, serious, and treatable
- Functional ability and safety review — fall risk, hearing, home safety, and ability to manage daily activities
- Complete medication review — every prescription, over-the-counter drug, and supplement, checked for interactions and opportunities to simplify
- A personalized prevention plan — a written screening schedule and checklist for the next 5–10 years, tailored to your risk factors
- Advance care planning — an optional, no-pressure conversation about your healthcare wishes, at your discretion
Why the AWV Matters So Much After 65
It catches cognitive decline early
The cognitive assessment is arguably the most valuable element of the visit. Early memory changes are easy to dismiss as “normal aging,” but early detection matters enormously: it opens the door to treatment options, safety planning, and support programs while the patient can still participate in decisions. For patients with a confirmed dementia diagnosis, it can also be the gateway to the CMS GUIDE Model — a Medicare program providing care navigation, a 24/7 support line, caregiver training, and up to $2,625 per year in respite benefits, at no additional cost to the beneficiary.
It untangles medication problems
Many seniors take five or more medications prescribed by different specialists over many years. The AWV medication review is a chance for one clinician to look at the whole picture — flagging interactions, duplications, and drugs that may no longer be needed. For older adults, careful “deprescribing” often improves energy, balance, and clarity.
It prevents falls before they happen
Falls are a leading cause of injury and loss of independence in seniors — and most are preventable. The AWV’s functional and safety review identifies balance problems, risky medications, and home hazards while there’s still time to act.
