What Is the CMS GUIDE Model? Free Dementia Care for Medicare Patients Explained

If you or a loved one has been diagnosed with dementia, you may feel overwhelmed by the challenges ahead. Managing medications, coordinating doctor visits, ensuring home safety, and supporting a caregiver who is stretched thin — it can feel impossible to do it all. The CMS GUIDE Model was created to change that.
GUIDE stands for Guiding an Improved Dementia Experience. It is a CMS Innovation Center program that provides comprehensive, team-based dementia care to Medicare beneficiaries — at absolutely no cost to the patient or their family.
How the GUIDE Model Works
The GUIDE Model assigns every enrolled patient a dedicated care team led by a geriatrician. This team includes a Care Navigator — a single point of contact who coordinates all aspects of the patient’s care journey. Think of the Care Navigator as your personal guide through the healthcare system, connecting you with the right resources at the right time.
Unlike traditional fee-for-service Medicare, where families often coordinate care on their own, the GUIDE Model wraps services around the patient and caregiver as a package. CMS pays the practice a monthly care management fee to provide these services, which means there are no copays, deductibles, or out-of-pocket costs for GUIDE services.
What Services Does GUIDE Provide?
The GUIDE Model provides a comprehensive set of services that address both the patient’s medical needs and the caregiver’s wellbeing. Here is what enrolled families receive:
Comprehensive Care Assessment and Planning: The care team conducts a thorough evaluation of the patient’s cognitive status, medical conditions, medications, home safety, and social support. Based on this assessment, an individualized care plan is created and updated regularly as needs change.
Dedicated Care Navigator: Your Care Navigator coordinates appointments, manages referrals to specialists, connects families with community resources like meal delivery and transportation, and serves as your go-to contact for any questions or concerns.
24/7 Support Line: Dementia crises do not follow business hours. The GUIDE Model includes round-the-clock access to clinical support for urgent behavioral changes, caregiver emergencies, or medical concerns that arise at night or on weekends.
Respite Care Benefits: Unpaid caregivers — typically spouses or adult children — receive up to $2,500 per year in respite care services. This can be used for in-home aides, adult day center programs, or short-term facility stays so caregivers can take essential breaks without worrying about their loved one’s safety.
Caregiver Education and Training: Evidence-based training programs help caregivers develop skills for managing challenging behaviors, reducing stress, and maintaining their own health while caring for someone with dementia.
Home Safety Modifications: In select cases, the program provides up to $1,000 for home safety assessments and modifications such as grab bars, door alarms, and fall prevention measures.
Who Is Eligible for the GUIDE Model?
Eligibility for the GUIDE Model is based on a few straightforward criteria established by CMS:
You must have a confirmed diagnosis of dementia (Alzheimer’s disease or a related dementia). This includes vascular dementia, Lewy body dementia, frontotemporal dementia, and mixed dementias.
You must have traditional Medicare (Medicare Part A and Part B). Medicare Advantage plans are not eligible for the GUIDE Model at this time.
You must live in the community — meaning a private home, assisted living facility, or memory care unit. Patients in long-term skilled nursing facilities are not eligible.
You cannot be enrolled in PACE or hospice. These programs have their own comprehensive care structures that overlap with GUIDE services.
There is no age requirement — any Medicare beneficiary with a dementia diagnosis who meets these criteria may enroll.
Why Was the GUIDE Model Created?
More than 6.7 million Americans currently live with Alzheimer’s disease, and that number is projected to reach nearly 13 million by 2050. Dementia is the most expensive condition in the United States, costing over $360 billion annually in healthcare expenditures alone.
Despite these staggering numbers, Medicare has historically lacked a dedicated care model for dementia. Families have been left to piece together care on their own — often leading to unnecessary emergency room visits, premature nursing home placement, and severe caregiver burnout. The GUIDE Model was launched in July 2024 to address these gaps by providing a coordinated, proactive approach to dementia care over an 8-year period (2024–2032).
