Florida Elder Care is now a participating practice in the CMS ACCESS Model — Advancing Chronic Care with Effective, Scalable Solutions — a 10-year innovation initiative from the CMS Innovation Center. As of July 5, 2026, we are actively enrolling.
The ACCESS Model shifts chronic disease care toward technology-enabled delivery and outcome-aligned payments, rewarding measurable improvements in patient health rather than volume of services. For the seniors and families we serve, that means more coordinated, more connected care.
Through the model, our team supports patients across four chronic-condition focus areas: hypertension, diabetes, chronic musculoskeletal pain, and behavioral health. Care includes remote patient monitoring, telehealth, and continuous digital engagement — reaching patients across Florida, from our Loxahatchee headquarters to Miami-Dade, Broward, Orlando, Tampa Bay, Jacksonville, and beyond.
For referring clinicians: the model provides regular electronic updates on your patients, and participating providers may qualify for an ACCESS co-management payment for reviewing those updates and coordinating care.
