What Is the ACCESS Model?
The ACCESS Model is a new program from the CMS Innovation Center — the same part of Medicare behind the GUIDE dementia care program we already offer. It pairs your Medicare coverage with technology-supported, team-based care for chronic conditions: remote monitoring where appropriate, structured follow-up between visits, and a care team that tracks your progress instead of waiting for the next appointment.
For decades, chronic condition care has meant a fifteen-minute office visit every few months and a lot of guesswork in between. ACCESS is designed to close those gaps.
The Four ACCESS Tracks
1. Early cardiometabolic risk — for patients with hypertension or prediabetes, focused on preventing progression before organ damage takes hold.
2. Cardiovascular-kidney-metabolic care — for patients already managing kidney, heart, or metabolic disease, with closer monitoring and coordinated specialty input. As a nephrologist as well as a geriatrician, this track is personal to me: I have spent my career watching kidney disease get caught too late.
3. Musculoskeletal care — for chronic back, knee, and hip pain, emphasizing function and evidence-based pain management over a prescription-first approach.
4. Behavioral health — for depression and anxiety, conditions that affect a large share of older adults and too often go untreated because access to care is hard.
Who Is Eligible?
Medicare beneficiaries whose diagnoses fit one of the four tracks. Eligibility depends on your specific diagnosis and coverage details — our team can confirm yours in a few minutes by phone.
What Does It Cost?
ACCESS services are billed through Medicare. Depending on the service and your coverage, standard Medicare cost-sharing may apply — our team confirms your eligibility and walks you through any costs before you enroll, so there are no surprises.
