GUIDE Program Eligibility
To determine if this program is the right fit, please review the requirements below, which are set by Medicare:
1. What is the required diagnosis?
You or your loved one must have a diagnosis of dementia (Click here to begin your preliminary assessment)
2. What insurance is needed?
Enrollment in Original Medicare (both Part A and Part B) is required. Please note: Medicare Advantage plans are not eligible for this program
3. Where must the individual live?
The program is for those living in the community, such as a private home, assisted living facility, or memory care units. It does not cover individuals in a skilled nursing home
4. Are there any other program exclusions?
Yes, individuals currently enrolled in a PACE program or receiving hospice care are not eligible

You will be paired with a Care Navigator who coordinates all aspects of your care, from doctor’s appointments to community resources like transportation and meals

We conduct thorough assessments to create a personalized, person-centered care plan that evolves with your needs
3. 24/7 Access to Support
You and your caregiver have around-the-clock access to a helpline for immediate guidance and support for urgent needs

Coordinated care helps to reduce emergency room visits, hospitalizations, and the need for long-term nursing home placement
For Family Caregivers
The GUIDE program is designed to reduce the significant strain and stress that often comes with caring for a loved one with dementia:

Access up to $2,500 worth of respite care services in 2025. This allows you to hire temporary in-home help, use an adult day center, or utilize an overnight facility to take much-needed breaks from your caregiving duties

Access evidence-based training programs that provide best practices and skills for caring for a loved one with dementia, helping you manage challenging behaviors and reduce stress
