- Our team offers an initial comprehensive geriatric assessment to develop a personalized annual care plan. Our goal is to help you achieve optimal health outcomes and prevent ER visits or hospitalizations.
- With your permission, we can act as your health surrogate and work with other healthcare providers to make informed decisions that benefit you.
- We will conduct scheduled house calls to reassess and adjust your care plan as needed based on any changes in your health status.
- Our dedicated team members are committed to providing comprehensive care for our patients. In the event that you are hospitalized, we maintain a detailed database by obtaining records from the hospital and other consultants involved in your care.
- As a part of our commitment to your health, our team will review and update your medication list every month. This ensures that you receive the correct dosage of all medications and that any potential drug interactions are avoided.
